Flu Shot Clinics: Online Consent Deadline TODAY

The Sevier County Health Department and the Sevier County School System have partnered to provide seasonal influenza (flu) vaccinations at your child’s school. The Clinic will be at SHS on October 26th. 

If you want your student receive the shot, you must complete the online consent form TODAY: click the link HERE to do this. Be sure to sign the online consent as this will be your permission for your student to receive the vaccine. A separate online consent form is required for each student you would like to receive the vaccine. If you have multiple children, simply click on the link again and complete for each child.

If your child is covered by insurance, including TennCare and private insurance, the County Health Department will file a claim with the insurance plan and receive reimbursement directly from the insurance plan. If your child is uninsured, there is a cost up to $20. If you would like to be considered for a sliding scale reduced fee, please contact the Health Department at 865-453-1032 to provide your family size and income. Do not send money to the school.
