Flu Shot Clinics: Parent and Student Information

Parents, 

If you like for your child to receive the flu vaccine at school, please completely fill out the electronic consent using the following: REDCAP link HERE. Be sure to sign the online consent as this will be your permission for your student to receive the vaccine. A separate online consent form is required for each student you would like to receive the vaccine. If you have multiple children, simply rescan the QR code and complete for each child. If later you change your mind and do not wish for your child to receive flu vaccine at the school, please notify the school prior to the clinic date!
